KMC Becomes First Hospital in Coastal Karnataka to Offer Enhanced Recovery for Joint Replacements


Mangaluru: Mangaluru KMC Hospital added another milestone to its credit and became the first Hospital in coastal Karnataka to offer enhanced recovery for joint replacements. 100% of Patients are made to walk on the same day afetr a primary Joint Replacement. KMC Hospital has several successful joint replacements to its credit owing to the establishment of a specialist centre with surgeon specializing in redo and complex cases , having performed complex surgeries and have given new lease of life to several patients.

Failure in joint replacements is not uncommon. Ideally a joint replacement if done properly should last more than three decades. Failure is often attributed to the quality of implants. People fall for latest expensive implants rather than focusing on consistent and viable techniques in theatre and recovery that suit patients best. Most of modern prostheses of all cost ranges last more than 25 years. Early failure is usually due to operation technique faults, infection or complications like fracture/ dislocation, and others.

Late failure is related to wear out of material of the implant. Patients who have had a joint replacement at ages younger than 60 years are more likely to experience this. Both types of failure can be managed with novel reconstruction techniques, in order to retain patient’s quality of life. It’s imperative to have a focused specialty unit for joint replacement that caters to the requirements of such surgeries. It flows into the fact that a unit routinely treating failures would be able to see it through flawlessly in primary straightforward cases. .

Speaking on the occasion Dr.Yogeesh D Kamat, Consultant Hip and Knee Surgeon, KMC Hospitals, Mangaluru said, “Infection in relation to joint replacements is still the most dreaded cause of failure or joint replacement surgeries. Infection has to be curbed immediately when detected. If a bug comes into contact with a metal implant and stays there for a few weeks, it sticks there such that any wash or antibiotic then cannot act. It then slowly creates pus and cavities in the bone around the metal and loosens the metal implant. Such infection can grow slowly over months without being obvious outside. Dressing and small surgery to wash pus would give just temporary relief with the problem getting worse inside. The longer it stays, the more difficult it becomes to eradicate. “

Ram ( name changed) had a successful hip operation over 30 years ago, which was good initially, but had worn out over the last few years, with inability to walk beyond a few meters and leg shortening of 4 cm. This was a case of late failure described above. Despite visiting several centres in the whole of South India and seeking opinion for his condition his prayers were answered at KMC Hospitals. Ram underwent a successful redo surgery at KMC Hospitals that involved reconstruction of his pelvis bone with bone from a bone bank and complex implants.

Dr Yogeesh D Kamat, said, “We had another adolescent case of Ms Ramya (name changed) had a condition that destroyed the lower end of her thigh bone. We were able to remove 9 inches of the thigh bone and make her walk again by making a customised prosthesis for her, that included resurfacing of her knee joint too. This was done in teamwork with our Paediatric Oncology service under Dr. Harsha Prasada”

He further added, “Revision surgeries are an increasing burden and are complex, as it requires precision, expertise and a bigger Hospital set up like KMC that offers quality and good teamwork between specialties. One can hence never undermine the importance of early detection of arthritis, and Joint Preservation intervention at younger ages, that prevents the requirement of Joint Replacement altogether or delays it to an age that chances of failure are reduced. I believe it is better to be recognised as a Knee & Hip Joint specialist rather than as a Replacement Specialist, as I’m not biased to replace joints but am equally passionate about preventing joint replacement in the younger population.”
